Guinea Pig Boarding Request Form

Please note submitting this form does not guarantee booking. We will review our availability and CONFIRM or DENY the request via email within 24-48hrs.

DEPOSIT:
  • If approved a deposit equal to 1 day of boarding will be due, this deposit is refundable up to 1 week before drop-off date.
  • If boarding during holidays or school break a 50% deposit will be due, this deposit is refundable up to 2 weeks before drop-off date.

 

We reserve the right to decline boarding if your rabbit is showing signs of illness or is infected with parasites at drop-off.

* include favorite food, treats, toys and any medical or behavioral concerns.
* describe their daily diet including how many times a day they are fed; types and amount of greens, vegetables and fruits; types and amount of pellets, treats and supplements.
* list any medications that your rabbit is currently taking and the condition. Type “N/A” if none.
